Apply for an assessment of your Wlz insurance position

If you want to know whether you are insured under the Wlz scheme, for example because you have received a letter from the CAK or because your health insurer needs an official decision from the SVB, you can apply to us for an assessment of your Wlz insurance position.

Can you apply for a Wlz assessment for someone else?

You can only apply for a Wlz assessment for yourself or for your children under the age of 18.

Your partner or any children over the age of 18 will need to apply for a Wlz assessment themselves. 

  1. Do not apply more than 8 weeks in advance

    If you are going to do an internship outside the Netherlands or a round-the-world trip, please do not submit your form earlier than 8 weeks before you leave. This is because we cannot take a decision until your situation has actually changed. We will send you our decision when we are sure that you have started your internship or round-the-world trip. 

    If you have received a letter from the CAK, you should send your application immediately.

    Please note that it can take up to 8 weeks for us to process your application.

  2. Explain the details of your situation

    Explain why you need a decision and describe your situation in as much detail as possible.

  3. Decisions covering a certain period 

    If you need a decision for a particular period, please give the period on your application. 

  4. Enclose documentary evidence

    Enclose documentary evidence with your application. For example, documents showing your work activities as a self-employed person, or as an au pair or intern. You will see what documents you need to send when you fill out the application form.

  1. Online

    Go to ‘Login’ and choose how you want to log in to My SVB. You can then submit your application quickly and easily.

  2. Sending a form by post

    Download the form and fill it in on your screen. Then print the form, sign it and send it to us by post.

  4. Cross-border teleworking

    If you are teleworking across the border, it could affect your social insurance, including your Wlz insurance. As from 1 July 2023, the EU will go back to using the normal European rules to determine the applicable legislation. You can only request an exception to these rules on the applicable legislation if you meet the necessary conditions.

    Check whether you meet the necessary conditions on the page ‘Cross-border teleworking’. If you do not meet these conditions, fill in the application form for an assessment of your insurance position under the Wlz scheme.

If you wish to apply for a Wlz assessment for a child under 18, you will have to send an application form by post.

Children over the age of 18 must apply for a Wlz assessment themselves.
